ABOUT THIS FORM
This form may be used by members of the UWL community, and visitors, to report information related to alleged violations of UWL's Sexual Violence and Sexual Harassment Policy. The University of Wisconsin-La Crosse is committed to preventing and responding to sexual misconduct to ensure a safe and equitable educational environment for all. The University prohibits all forms of sexual misconduct, including but not limited to: Sexual assault, sexual violence, domestic violence, dating violence, stalking, sexual harassment, sexual exploitation, and discrimination based on gender identity or sexual orientation.
WHO CAN USE THIS FORM?
Anyone can use this form. (Note: All university employees, including faculty and staff, must report incidents to the Title IX Coordinator if they receive notice of an incident of sexual or gender-based misconduct, unless they are a Confidential Resource).
WHAT HAPPENS WHEN I SUBMIT MY REPORT? WILL I BE CONTACTED?
Submitted reports are delivered to the Title IX Coordinator and additional recipients, for review.
- The Title IX Coordinator will work to identify the Complainant (recipient of behavior) if that information is not already provided in the report.
- If the Complainant's information is noted in the report, the Title IX Coordinator will respond to you, the Reporter, with an email to let you know the report was received by the Office of Title IX and Compliance.
- The Title IX Coordinator will outreach to the Complainant to offer support, resources, and explain their rights.

The University supports all persons who are impacted by sexual misconduct. You may talk with the Title IX Coordinator or Violence Prevention Specialist to learn what resources are available to you.
