Online Enterprise Work Order Request
The link below provides access to an online form to submit your Data Requests. This can be used to submit requests to run known programs or submit requests for data extracts, etc. The forms help us speed your request into our system.
How it Works (Please Read the First Time Carefully)
Click on the link below and complete all fields in the online form. When you click Submit, we will receive the request and a confirmation email will be sent to your UW-L email address shortly after we receive it. Data and Program Request Form: Use for all Enterprise Data Requests that have a specific program name and those that do not have a specific program name associated with them such as query requests.
Please Note: Clicking on the link above will take you to Web Help Desk. To submit a data request you will need to login to Web Help Desk with your NetID Account. You will then need to select the following Request Type.
