How to earn a $150 prepaid card*
Complete three activities before October 11, 2024:

  • HEALTH ASSESSMENT:Get a personalized health report in 15 minutes.
  • HEALTH CHECK: This can be a biometric screening, dental check or one coaching session.
  • WELL-BEING ACTIVITY: Listen to two Well Wisconsin Radio podcasts from season 3, attend a virtual Coaching Class, or take part in other activity options that match your goals.

Not sure which activities you’ve completed so far? Check the Rewards page to see your progress or Already up to date on your screenings?  Download and complete the Health Care Provider Form to get credit for knowing your numbers. Just click the Benefits tab, then the Biometric Screening card for details.

*The Well Wisconsin incentive program is a voluntary program available to employees, retirees and spouses enrolled in the State of Wisconsin Group Health Insurance Program, excluding Medicare Advantage participants who have incentives available through their health plan. The Well Wisconsin incentive will automatically be issued to eligible participants upon completing the applicable activities. All wellness incentives paid to participants are considered taxable income to the group health plan subscriber and are reported to their employer, who will issue a W2. In some cases, the Wisconsin Retirement System acts as the employer. Retirees, continuants and their spouses will have some taxes withheld from the incentive amount earned.
