Diabetes Management
The It’s Your Health: Diabetes program provides one-on-one diabetes management support with a nurse coach and reduced copays for diabetes-related prescriptions for members who enroll in the IYC Health Plan and Access Plan. Members who enroll in a high deductible health plan can still receive one-on-one support but will be responsible for the pharmacy costs as outlined in their benefits materials.
