Disclose outside activities that may conflict with obligations at UW-La Crosse.
Annually, most employees classified as Limited, Faculty, and Academic Staff are required to disclose any outside activities that may pose a conflict with their official obligations at UW-La Crosse.
For the 2021 submission, (between March 8 and April 30), HR is working to have these forms automated and placed in the MyUW Portal so that the paper copy will not be required in the future. No action is required at this time.
Additional communications will be forthcoming regarding the submission process and where to find the form.
