Thank you for your interest in the Clinical Laboratory Science Program at the University of Wisconsin-La Crosse. On this web page you will find the application form for the Professional Program. Eligibility to apply to the Professional Program: Students are required to have completed the bulk of the pre-clinical coursework and at least one semester at UWL at the time of application, or to have completed a Bachelor of Science degree in an associated field. GPA of 2.75 or above is required. GPA's below 2.75 will not be considered. All program-required science/math courses must be a "C" or better. The application deadline (for all application materials) is February 15. Please refer to the instructions when filling out your application. Acceptance into the CLS program is competitive as practicum positions at affiliated locations are limited. Even though a student may meet the minimum requirements, their acceptance is not guaranteed due to program size limitations.  Screened applicants will be interviewed in March and offers of admission, alternate admission and rejection will be made by the end of March. Classes in the Clinical Laboratory Science Program will begin that upcoming Fall.

Affer acceptance to the CLS major, you will be required to make a separate application for acceptance in the Clinical Phase at one or more of the accredited programs affiliated with UWL. The application for the clinical phase is submitted early in the Fall semester, and you will be notified if you are accepted in the same semester.

The UW-La Crosse Clinical Laboratory Science Program is an academic affiliate of programs accredited by the National Accrediting Agency for Clinical Laboratory Sciences. The affiliates are: 

  • Ascension/St. Elizabeth Hospital Laboratories; Appleton, WI
  • Marshfield Laboratories; Marshfield, WI
  • Aspirus Wausau Hospital Laboratories; Wausau, WI
  • Hennepin Healthcare Laboratories; Minneapolis, MN
  • Mayo Clinic; Rochester, MN
  • University of North Dakota (University affiliation); Grand Forks, ND

Upon completion of the program, graduates are eligible to sit for the national certification examination.
